New hospital-initiated medication review pathways
AMA members who treat elderly patients should be aware that the Society of Hospital Pharmacists of Australia has released a new resource to support the transition of patients from hospital into primary or residential aged care.
The framework supports the identification and referral by hospital-based clinicians of high-risk patients to Home Medicines Review, Residential Medication Management Review and Hospital Outreach Medication Review pathways.
