GP care for patients with chronic conditions

The National Health Performance Authority this week released Healthy Communities: GP care for patients with chronic conditions in 2009-2013.  The report provides a useful contribution about access to care for patients with chronic illness and how they are managed. The report is designed to help inform the yet to be established Primary Health Networks. 

The AMA has always said that PHNs should focus on gaps in service delivery and providing support for GPs in caring for patients, and this report may provide some useful data that can assist PHNs in this role.
