AMA position on effective transfer of care
The position statement outlines requirements for appropriate and effective transfer of care arrangements between the GP and the hospital and vice versa.
The AMA’s position statement on General Practice/Hospitals Transfer of Care Arrangements has been revised with guidance provided by the AMA Council of General Practice and input from the Council of Public Hospital Doctors and federal councillors. The revised position statement was approved by Federal Council at its August meeting.
Key changes include clarifying statements regarding referrers’ responsibilities, amendments to emphasise the need for involving the patient’s GP in planning for management and discharge from hospital and for interoperability standards to facilitate real time transfer of information, to promote greater collaboration between hospitals and GPs.
